The Coach and the Team Manager should keep a copy of the signed Medical Release Form in case of an emergency. They are also mandatory at Check-In for all FYSA tournaments.
Note to Team Managers: Please check on the Check-In requirements posted by each tournament. They may vary and require Medical Release forms to be notarized. Some tournaments require that you use their version of the medical release form provided on their website. Make sure you follow each tournament’s specific requirements.
