Medical Release Form
Medical Release forms should be kept by the Coach and the Team Manager in case of an emergency. They are also mandatory at Check-In for all FYSA tournaments.
Note to Team Managers: Some tournaments are not requiring that Medical Release Forms be notarized. Please check on the Check-In requirements posted by the each tournament your team will be registering to be sure you satisfy those requirements. Some tournaments require that you use their version of the medical release forms provided on their website. Make sure you follow the specific requirements for that tournament.
